Verified Frontline Experience & Academic Background

Rooted in Frontline Youth Advocacy

Yosmerys holds a Bachelor of Criminology (Venezuela) and a Diploma of Counselling (Australia), bringing over eight years of hands-on frontline experience across Western Sydney Out-of-Home Care (OOHC), Youth Justice case management, and specialist NDIS disability support.

Having spent years working directly with young people facing complex trauma, family breakdown, and social disconnection, Yosmerys established Supporting Communities Australia to provide a different kind of support: one grounded in consistent rapport, dignity of risk, and real-world skill acquisition.

Practice Standards & Human Rights

Our Core Principles & Rights-Based Care

Every interaction is guided by choice, control, and respect for young people.

Dignity of Risk

We believe young people learn best by trying, reflecting, and adjusting in safe environments. Overprotection limits growth; dignity of risk builds genuine self-reliance.

Choice & Control

Young people actively co-design their support plans, select activities, and direct how mentoring occurs. We support them to exercise choice at every stage.

Supported Decision-Making

Rather than deciding on behalf of a young person, we provide clear information, discuss options, and assist them to make their own informed choices.

Child Safe Standards & NDIS Quality Safeguards

Audited & Registered NDIS Support Provider

Supporting Communities Australia strictly enforces NSW Child Safe Standards, national NDIS Safeguards, and mandatory reporting obligations. All mentors undergo rigorous Working With Children Checks (WWCC), NDIS Worker Screening Checks, comprehensive background verification, and continuous trauma-informed supervision.
